Students with either seen (physical, mobility, visual or hearing impairment) or unseen (long term medical condition, mental health challenges or hearth condition) disability is require to provide a letter either from a psychologist, psychiatrist or a medical specialist that includes the following details: 

• What is the diagnosis of the disability or medical condition?
• When the impairment or medical condition was diagnosed?
• How long is the impairment or medical condition is going to last?
• How the medical conditions or disability will affects day to day activities?
• Any side effects of any medications or treatment?

Student with learning differences such as Dyslexia, Asperger’s Syndrome or ADD/ ADHD will require an assessment report from an educational psychologist or a specialist in special needs.
