Medical Evidence Policy
Medical evidence is required in order for the disability advisory services to better understanding of student’s disability and to provide proper supports.
Students with either seen (physical, mobility, visual or hearing impairment) or unseen (long term medical condition, mental health challenges or hearth condition) disability is require to provide a letter either from a psychologist, psychiatrist or a medical specialist that includes the following details:
- What is the diagnosis of the disability or medical condition?
- When the impairment or medical condition was diagnosed?
- How long is the impairment or medical condition is going to last?
- How the medical conditions or disability will affects day to day activities?
- Any side effects of any medications or treatment?
Student with learning differences such as Dyslexia, Asperger’s Syndrome or ADD/ ADHD will require an assessment report from an educational psychologist or a specialist in special needs.
